What are Dual Eligible Special Needs plans?

Dual Eligible Special Needs plans (D-SNP) combine Medicare and Medicaid benefits. These plans give you extra help and care when you need it. 

Mass General Brigham Health Plan has two D-SNP plans. They come with support from a care coordinator and a team of specialists. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental and vision care, and transportation to your appointments. 

Are you 65 or older?

Mass General Brigham SCO is a health plan for people ages 65+. This plan supports independent living and includes a fitness benefit to help you say active.

Mass General Brigham One Care is a health plan for people ages 21 to 64. This plan includes behavioral health support to help you achieve your recovery goals
