Mass General Brigham SCO is a health plan for people ages 65+ who have MassHealth and Medicare. It’s easy to use because it combines the benefits you already have into one health plan with one card.

 

It also includes support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental and vision care, and transportation to your appointments.

With our SCO plan, you’ll have $0 copays for doctor visits, hospital stays, and urgent care. You also get more benefits that go beyond MassHealth and Medicare, including:

Get a quarterly allowance for the over-the-counter products you use most.

Members get unlimited free rides to your medical appointments.

How to enroll

If you're ready to enroll in Mass General Brigham SCO, you can connect with a Plan Associate. A member of the team will help you review plan benefits and determine your eligibility. If you are eligible, they can help you complete an application by telephone.

When you enroll, you'll be assigned a care coordinator. This person will work with you, your primary care provider, and your care team. Together, they will make a care plan for your personal health needs. If you need more support, your care team might also include other specialists.

Pharmacy benefits – You'll pay $0 for prescription drugs on our SCO plan drug list. You can check the drug list, also known as a formulary, on our pharmacy benefits page.

Over-the-counter (OTC) benefit allowance – You'll get a $275 quarterly allowance to spend on the OTC products you use most, like pain relievers, cold and allergy medications, vitamins and supplements, first aid supplies, and more. SSBCI-approved members may use up to $125 of their $275 quarterly allowance to buy food and personal care items.

Dental – Our SCO plan has a $0 copay for cleanings and exams, X-rays and fillings, extractions and root canals, and dentures and denture fittings. You can find covered dentists by checking the dental network, and a list of covered dental services in the dental plan benefit summary.

Vision – Annual eye exams are $0 for members. You’ll also get $200 a year to buy glasses and contact lenses. Use the online vision provider locator to find covered vision providers.

Hearing – Annual hearing exams are free and hearing aids are covered.

Stay active at home or on-the-go with your fitness benefit. Your benefit includes a $250 yearly allowance to use on wellness purchases like memberships at participating gyms, fitness classes, workout equipment, and virtual classes.

Medical appointment transportation – Our SCO plan includes unlimited rides to medical appointments.

Non-medical transportation - SSBCI-approved members have access to four one-way non-medical trips connected to their care plan each month.

Personal help programs – You may be eligible for long-term support services (LTSS). These services provide hands-on care in your home. You can get help with activities like getting dressed, eating, taking a bath, and doing laundry.

  • Social, behavioral, and other specialized supports

Pharmacy benefits

You'll pay $0 for prescription drugs on our SCO plan drug list. You can check the drug list, also known as a formulary, on our SCO and One Care Pharmacy Benefits page.
