Mass General Brigham One Care is a health plan for people ages 21 to 64 who have MassHealth and Medicare. It’s easy to use because it combines the benefits you already have into one health plan with one card.

 

It also includes support from a care coordinator and an integrated care team. This team will work with you and your doctors to meet your care needs. You'll also get other benefits like dental and vision care, and transportation to your appointments.

With our One Care plan, you’ll have $0 copays for doctor visits, hospital stays, and urgent care. You also get more benefits that go beyond MassHealth and Medicare.

Transportation

Members get unlimited free rides to your medical appointments.

How to enroll

If you're ready to enroll in Mass General Brigham One Care, you can connect with a Plan Associate. A member of the team will help you review plan benefits and determine your eligibility. If you are eligible, they can help you complete an application by telephone.

    When you enroll, we will assign a care coordinator to help you get the care you need. This person will work with you, your primary care provider, and your care team. Together, they will make a care plan for your personal health needs.

    You can also call our 24/7 advice line to speak with a registered nurse about any of your healthcare concerns.

    If you need more support, your care team might also have:

    • A long-term services (LTS) coordinator who can help you find resources in your community
    • A pharmacist who knows your prescription medications
    • Specialists and other health care providers for your health conditions
    • Social, behavioral, and other specialized supports

    You'll pay $0 for prescription drugs and qualifying over-the-counter products on our One Care plan drug list. You can check the drug list, also known as a formulary, on our SCO and One Care Pharmacy Benefits page.

    Dental – Our One Care plan has a $0 copay for cleanings and exams, X-rays and fillings, extractions and root canals, and dentures and denture fittings. You can find covered dentists by checking the dental network, and a list of covered dental services in the dental plan benefit summary.

    Vision – Vision services, including exams and glasses, are covered every two years. Medically necessary contacts may be approved by your provider in place of glasses. Use the online vision provider locator to find covered vision providers.

    Hearing – Annual hearing exams are free and hearing aids are covered.

    Members can get counseling and treatment for:

    • Substance use disorders
    • Depression, including postpartum depression
    • Eating disorders
    • Suicidal thoughts
    • Attention-deficit/hyperactivity disorder (ADHD)
    • Many other mental health conditions

    Transportation – One Care includes unlimited rides to medical appointments and two round trips per month up to 50 miles each way to non-medical destinations connected to your care plan.

    Personal help programs – You may be eligible for long term support services (LTSS). These services provide hands-on care in your home. You can get help with activities like getting dressed, eating, taking a bath, and doing laundry.

    Mass General Brigham One Care is a Dual Special Needs Plan (D-SNP) that contracts with both Medicare and MassHealth (Medicaid) to provide benefits of both programs to enrollees. Enrollment in the plan depends on the plan’s contract renewal with Medicare. This plan is a voluntary program that is available to anyone age 21 - 64 who qualifies for MassHealth Standard or CommonHealth and Original Medicare and does not have any other comprehensive health insurance, except Medicare.

    Mass General Brigham SCO (Senior Care Options) is a Dual Special Needs Plan (D-SNP) with a Medicare contract and a contract with the Commonwealth of Massachusetts Medicaid program. Enrollment in the plan depends on the plan’s contract renewal with Medicare. This plan is a voluntary program that is available to anyone 65 and older who qualifies for MassHealth Standard and Original Medicare and does not have any other comprehensive health insurance, except Medicare.
